Job Description

Are you a qualified Social Worker looking to make a real difference in how patients transition from hospital back to their communities? Newport City Council is recruiting Adults Social Workers to join the Newport Hospital Discharge Service, based at the Royal Gwent Hospital in Newport, South Wales.

This is a genuinely exciting opportunity to be part of the Home First Service — a team that has established itself as instrumental in shaping discharge pathways and improving the hospital experience for citizens across acute sites in the region. Your work will directly influence how patients leave hospital safely and at the earliest opportunity, improving outcomes for individuals and their carers.

Key responsibilities include working with individuals, families, and carers to coordinate appropriate discharge arrangements; collaborating within a multi-disciplinary team of qualified and non-qualified staff; managing and overseeing discharge requirements; and engaging with key stakeholders to achieve positive outcomes.

Please note: this role is predominantly site-based at the Royal Gwent Hospital, with limited home working subject to management approval.

The role is offered on a locum basis at £20.98–£28.01 per hour via Prism, with a start date of 18 May 2026. Candidates must hold a recognised Social Work qualification and be registered with Social Care Wales.
