Job Description

An exciting opportunity has arisen for a dedicated SEND Key Worker to join a specialist team based at Delamere House, Crewe (CW1 2JZ), offering a competitive rate of £25.18 per hour.

As a SEND Key Worker, you will play a vital role in supporting children and young people with Special Educational Needs and Disabilities, helping them and their families to navigate services, access the right support, and achieve meaningful outcomes. You will act as a consistent point of contact, coordinating across education, health, and social care to ensure a joined-up approach to each young person’s plan.

Key responsibilities will include maintaining a caseload of children and young people with SEND, working in partnership with families, schools, and multi-agency professionals, contributing to Education, Health and Care (EHC) plans, and advocating on behalf of young people to ensure their needs are met effectively.

The successful candidate will hold a relevant qualification in social care, education, health, or a related field, and will have demonstrable experience of working with children and young people with SEND. Strong communication and organisational skills are essential, as is the ability to build trusting relationships with families and professionals alike. An Enhanced DBS check will be required for this post.
