Job Description
1. Manage the designated areas of social care, ensuring improved outcomes for people and provide good value for money that best meet the needs of the localities of Calderdale. 2 2. Contribute to the redesign, improvement and development of services that build on best practice between preventative and early intervention approaches alongside personalised and person centred care and support models that maximise independence and increases choice and control. 3. Create a culture that ensures service delivery which reflects the different needs of Calderdale Cares localities and the relative strengths of service providers and community assets. Promoting creativity and innovation through personalised approaches to meeting individual’s social care needs, re-connecting them to their community and building on natural networks of support. 4. Ensure that teams deliver an effective, efficient service aligned to local needs, strategic plans and our statutory requirements. 5. Build collaborative relationships with partners across the Council, Health, Third Sector, Community Groups and our provider market. 6. Contribute to the identification of gaps in service provision and ensure services are fit for purpose. 7. Identify opportunities for further transformation through broadening our approach to collaborative working and integration across health and social care. 8. Manage multidisciplinary professional teams. 9. Responsible for delivery of budget action plans, budget management and budget settings. 10. Responsible for the achievement, performance, outcomes and objectives for a designated area. 11. Manage all aspects of risk and be accountable for the safety of staff, service users and contractors in accordance with all statutory obligations and relevant health and safety policies. 12. Lead, co-ordinate and ensure the effectiveness of activities for the purposes of safeguarding and promote the welfare of adults at risk of abuse in Calderdale. 13. Act as part of Extended Leadership Team to establish shared responsibility for the corporate management of the Directorate, supporting cross directorate leadership and change.
