Job Description

Senior Occupational Therapist with own vehicle required to support reablement pathway & crisis intervention / hospital avoidance and long term complex needs. Must be competent in Moving and handling and familiar with equipment used – hoists, stand hoists, returns & bed management systems in particular. Community experience essential as we are a community based team working directly with in house urgent care provisions.All candidates must have had the Covid-19 vaccination prior to being submitted to this order. Minimum 2 yrs post qualified experience Vale of York and surrounding Social work England registered Full Time Flexible and responsive for visits in Vale of York area occasionally Agency workers still to work from home Monday to Friday only 8 weeks initially with view to likely extension dependent on demand

Purpose of the Job

To contribute to the provision of Occupational Therapy Services within the Directorate in order to promote independence, well being and choice for people of all ages with a disability or long term condition.

Key Accountabilities

  • Specific Accountabilities
  • To meet the requirements of the professional regulator and to uphold the reputation of the profession and the Council.
  • To work within the Professional Code of Conduct of Occupational Therapists
  • To undertake activities to support CPD and participate in the student placement program as per HCPC requirements.
  • To keep up to date with professional developments in the field of Occupational Therapy and Housing
  • To manage and monitor a workload, organising work activities, Prioritising and delegating or referring on as appropriate.
  • To provide a co-ordinated approach to the assessment of need and produce outcome focused care plans.
  • To work in partnership with the people who use the service, their carers and other providers, respecting their views and promoting participation in decision making wherever possible whilst recognising the needs of all parties.
  • To undertake assessment of need of service users who require a complex assessment, co-ordinating those assessments where multi-disciplinary input is required in accordance with the Directorates protocols.
  • To respond to requests for assessment and provision of services in a timely manner.
  • To provide functional assessments within a variety of community based establishment eg persons home, Assisted Living Centre.
  • To undertake environmental assessments in relation to provision of equipment and management of service provision.
  • To make recommendations, based on need for the provision of services, aids to daily living and adaptations.
  • To ensure that recommendations are carried out, making appropriate use of specialist skills in other departments.
  • To ensure that any equipment supplied is fitted and demonstrated safely and clients instructed on the safe use of equipment.
  • To review the effect of OT intervention on client’s function i.e. were outcomes achieved to provide a sensitive, flexible and appropriate response to individual service users and their carers.
  • To be aware of carers needs and offer carers assessments as per Care Act legislation.
  • To undertake supervisory responsibility for Occupational Therapists, Assistant Care Co-ordinators, Team Assistants and students to include case load supervision and case closure.
  • To be responsible for obtaining regular supervision, evaluating practise, reflection and personal development.
  • To deputise for the Assistant Team Manager/Team Manager as required.
  • The post holder must have a full valid driving licence and use of vehicle insured for business purposes or must be able to make efficient and effective travel arrangements as required by the post.
  • To undertake a drivers permit test to enable the worker to drive DMBC vehicle to support the service and equipment assessment and provision.

· To participate in the Adult Care Duty System, including weekends.

· To maintain records and write reports in accordance with the Directorates Policy and Procedure

· To provide timely assessments and produce an agreed rehabilitation programme for individuals who require the services of the Living Independently Team in order to promote independence and reduce packages of care.

  • To keep up to date with changes in Assistive Technology and Telecare
  • To provide a co-ordinated approach to the assessment of needs and to develop individual packages of care
  • To undertake, monitor and evaluate the assessment of needs and to develop individual packages of care, baring in mind the philosophy of single handed care.
  • To be responsible for the design and costing of care packages, which are flexible and responsive to the needs or service users and their carers.
  • To be responsible for the safe handover of equipment, with the relevant documentation, demonstrations and training provided to the people using the equipment.
  • To undertake training as appropriate including Fieldwork Educators Course and participate in the clinical education of students
  • To liaise with other staff within Adult Social Care in matters relating to OT and to act in an advisory capacity to these teams
  • To maintain effective working relationships with internal and external providers and to convene and attend multi-disciplinary meetings as required
  • To keep up to date with professional developments in the field of Adult Services and to make appropriate recommendations to the Team Manager on the development or review of services
  • To contribute to service developments where appropriate
  • To develop a comprehensive knowledge of local resources in order that service users/carers are fully informed as to the choices of assistance available to them
  • To be a committed team worker and participate in working towards targets and monitoring and evaluating the work of the Directorate
  • Demonstrate effective IT skills – emails, word, excel, electronic case recording. Demonstrate an awareness of the appropriate Information Governance and data protection related to information sharing.
  • It is essential for the post holder to have experience in Reablement, urgent care and long term management in the community setting
  • To perform statutory duties appropriate to the post
  • Be flexible and reliable in regards to hours of work, including weekends and bank holidays; and in the approach to the role, in order to ensure that the strategic needs of the service and the statutory obligations of the Local Authority are met in providing the necessary requirements to the clients and the carers
  • To undertake any other duties appropriate to the post as required
  • This job description is provided to assist the post holder to understand what their duties are and may be amended from time to time without change to the level of responsibility appropriate to the grade of the post

· ADDITIONAL DUTIES

  • It is envisaged that all staff would participate in the core business of the Directorate This includes the following examples, the discharge from hospital, prevention of admission to hospital, maximising personal performance through rehabilitation and maintenance of long term care using the Single handed care philosophy. If circumstances required eg Emergency Plan staff would be required to undertake duties of a different nature, which could be implemented.
  • To arrange placements in residential and nursing homes following assessments, using the Directorates guidelines and with the agreement of the budget holder
  • To provide advice, support and counselling for service users and their carers who require such assistance within a geographical area or specialism
  • To be responsible for the implementation, monitoring and review, within the Directorates guidelines of Care Packages and to respond to changes through re-negotiation of the Care Package.

· HEALTH & SAFETY

  • You must comply with the requirements of the Health and Safety at Work Act 1974 and affiliated regulations. You are also required to comply with the Council’s Health and Safety Policies and take reasonable care for the health and safety of yourself and of other persons who may be affected by your acts or omissions. Where the Council has a statutory duty with regard to health and safety you are required to cooperate with the Council and it’s managers and officers so far as is necessary to enable the Council to meet that duty. This includes using equipment and substances in accordance with training and instruction and the reporting of serious dangers to your own or others safety.
  • As an employee you shall not intentionally or recklessly interfere with or misuse anything provided in the interests of health, safety or welfare.
  • You are required to ensure, so far as is reasonably practicable, that persons under your control are aware of, and comply with, their statutory health and safety duties and those set out within Council policies.

You must have:

  • A professional qualification and experience of working with Adults
  • Post qualified experience.
  • Enhanced DBS/Update service.
  • References required for the last 5 years of employment.
