Key Responsibilities: • Provide strong leadership and operational management to the Hospital Discharge and Urgent Care social care team. • Develop and implement strategic plans to improve service delivery, ensuring compliance with statutory guidelines and local policies. • Collaborate effectively with health partners, external agencies, and internal teams to drive seamless care transitions. • Oversee assessments, care planning, and risk management for complex cases. • Ensure the delivery of person-centred, strength-based approaches to social care. • Monitor and evaluate service performance, implementing improvement strategies where necessary. • Manage budgets effectively, ensuring resource allocation aligns with service priorities. Requirements: • Qualified Social Worker with current registration with Social Work England. • Substantial experience in adult social care, particularly in hospital discharge and urgent care settings. • Proven leadership experience, with a track record of managing social care teams effectively. • Excellent knowledge of relevant legislation, policies, and best practices in adult social care. • Strong communication, negotiation, and decision-making skills.

– Degree in Social Work
– SWE Registration
– Right to work within the UK
