Childrens Senior Practitioner (Qualified)

A position has arisen within Pathways, Leaving Care service and applications are invited from suitably qualified social workers to join this busy and highly motivated team. Pathways provides quality services to young people in accordance with the Children (Leaving Care Act) 2000 and the Children Act 1989, Guidance and Regulations volume 2 and 3. The roles and responsibilities will include: • Working with young people who are looked after and care leavers • Working with unaccompanied asylum seekers • Undertaking needs assessments and care / pathway plans • Working with young people leaving foster / residential care and promoting independent living skills • Carrying out reviews and planning meetings • Working with care leavers living independently • Offering support and befriending services • Duty work based at 3 Park Road North • Liaising with other professionals and other agencies Expected caseload is 20.
