Social Care assessments for Rapid Response patients

Commissioning of POC and bed based placements

Completion of MCA & BI

Working in an integrated, multi disciplinary, multi organisational team

Responding to people in crisis, within given service performance indicators of either 2 hours or 48 hours

Supporting and coaching a CCW

MUST HAVE:

- Post experience in Social Work

- SWE Registered

- Enhanced DBS
