We are seeking an enthusiastic and dynamic Social Worker to work within Manchester's Hospital Discharge to Assess service.
- Experience of hospital discharge is preferred.
- Understanding of Discharge to Assess model.
- Ability to work flexibly on a rota basis Monday - Sunday between 8 am - 8pm (35 hour week).
- Essential car user role.
- Experience of Liquid Logic preferred.

You must have:
- A professional qualification in social work and experience of working with Children, in particular previous experience of Assessment work
- Social Work England registration or Social Care Wales registration for North Wales.
- Post qualified experience.
