Location – Fylde & Wyre Interviews – ASAP SPA- triage for hospital – discharge Ability to apply the eligibility criteria under the Care Act Ability to complete Continuing Health Care Assessments / Reviews Ability to complete Mental Capacity assessments in line with the MCA 2005 Previous experience or knowledge of hospital discharge pathways See attached JD for full details Number of positions: 1
