Hospital Social Worker – Based at Wrexham Maelor Hospital (with possible work across County of Flintshire) 37 hrs for at least 3 months Candidates must be experienced in supporting hospital discharges and must also be registered with Social Care Wales. An opportunity has arisen for a Social Work Practitioner to support Adults in Flintshire, in order to facilitate timely hospital discharges. The post-holder will work within a multi-disciplinary team to provide a holistic assessment in the discharge process. Flintshire Hospital Social Work teams have access to various supporting services such as an in-house Reablement team, Community Resource team, Intermediate Care and therapeutic intervention for individuals. This role is fast paced and you will hold the case until the person is discharged to the ‘Discharge to Assess/Intake’ team with short term intervention. The ability to speak Welsh is desirable As this post will come into contact with vulnerable adults and/or children you will be subject to an enhanced disclosure and barring service check.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration.
  • Post qualified experience.
  • Enhanced DBS/Update service.
  • References required for the last 5 years of employment.
