We are seeking a dedicated and compassionate Social Worker to join our Community Discharge to Assess Team (D2A). In this role, you will be responsible for conducting comprehensive Care Act assessments for individuals who have been discharged from hospital into a temporary 24-hour care placement. Your primary objective will be to assess the individual’s care needs, identifying appropriate services, and coordinating the care and support. Knowledge and experience in the discharge to assess process is essential for this role.
Responsibilities:
Conduct Care Act assessments of individuals who have been discharged from the hospital and require community support.
Assess and evaluate the individual's physical, mental, and emotional needs, as well as their functional abilities, to determine the appropriate level of care and support required.
Collaborate with individuals, their families, and other healthcare professionals to gather relevant information and develop comprehensive care plans.
Provide advice, guidance, and emotional support to individuals and their families, helping them to understand their options and make informed decisions.
Liaise with relevant community resources, such as healthcare providers and voluntary organisations, to ensure the seamless provision of services.
Advocate for individuals' rights and entitlements, ensuring that they receive appropriate and timely support services.
Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with organizational and legal requirements.
Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services.
Requirements:
Bachelor's or Master's degree in Social Work
Valid social work registration.
Valid driver's license and own car with business insurance
Proven experience in conducting assessments and developing care plans.
Experience in Discharge to Assess is essential.
Sound knowledge of the Care Act, Mental Capacity Act and Best Interest Processes is essential for this role.
Knowledge and experience of Continuing Health Care processes and participating in DST's would be an advantage, but not essential.
Excellent communication skills, both verbal and written, with the ability to convey complex information in a clear and empathetic manner.
Effective organisational and time management skills, with the ability to prioritise tasks.
Strong interpersonal skills and the ability to build rapport with individuals, families, and multi-disciplinary teams.
Proficient computer skills, including experience with electronic record-keeping systems

You must have:
- A professional qualification in social work
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
