An exciting opportunity has arisen for permanent Lead Practitioners within the Award-winning Integrated Community Services (ICS) Team. The service encourages a ‘Home First’ approach, ‘right care, right time, right place’, working closely with our in-house CQC Registered reablement service, START and partner agencies. As a Lead Practitioner you will be actively involved in facilitating complex discharges from Acute and Community Hospitals within Shropshire and within neighbouring counties to repatriate Shropshire residents back home whilst also providing an Admission Avoidance service. The Lead Practitioner supports and mentors’ practitioners across the service to enable reflection, learning and to lead on implementation to support workers to adapt to change with an aim to improve practice. This is a varied role incorporating complaint investigations, targeted audit activity and training within a team where the remit constantly evolves to embrace change and meet the changing demands of the service. The job entails undertaking a wide range of duties, so a flexible approach to your work is required. The role includes working under pressure at times of high demands from our system partners and having a high level of personal drive. ? The lead practitioner has supervisory responsibility for all practitioners within their team. Whilst also undertaking yearly appraisals to ensure development and training needs of team members are identified and met ? Day to day management responsibility for operational activities within and across service area ? Support change and support practice development with team members ? Undertake HR related matters including, but not limited to recruitment and selection of staff, Management of Absence, Disciplinary, Capability and Grievance issues. ? Maintain emotional resilience in working with challenging behaviors and attitudes. ? To participate in the development of services with partner agencies. ? Recognise and respond to the need for change and actively encourage and support new approaches to service delivery. ? Ensure effective communication and consultation with partners, carers, service users and other stakeholders. ? Promote partnership working and integration within and outside of the council. The post holder will be expected to adopt a flexible attitude to the duties of the post. These may be varied after discussion between the post holder and management team to take account of the changing needs and development of the Service areas.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration.
  • Post qualified experience.
  • References required for the last 5 years of employment.
