Job Description

Rapid Response is a multi-disciplinary team of social workers, therapists, nurses, advanced practitioners, and social care workers. The team is an admission avoidance service responding within 2 hours to any health or social care crisis in the community. The team also support A&E to avoid admissions onto the wards.

The team works 3 shifts a week over 7 days, from 08:00-20:30. Weekend working is required.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration.
  • Post qualified experience.
  • References required for the last 5 years of employment.
