Job Description

We are seeking a qualified Adult Social Worker to work within Urgent Care and Discharge to assess and Community Flow This role will include some community visits to peoples home, but will on the whole be underthing assessments within our Discharge To Assess beds.

Excellent knowledge of the Care Act, Mental Capacity Act etc
Ability to write evidenced based assessments
Ability to work to deadlines.
Ability to manage a caseload in a fast paced environment.
Car user.
Support will be provided around practice and process, and supervision will also be provided.

Knowledge of Liquid Logic would be benifical.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration or Social Care Wales registration for North Wales.
  • Post qualified experience.
  • Enhanced DBS/Update service.
  • References required for the last 5 years of employment.
