An exciting opportunity has arisen for a Social Worker on the Manchester Case Management Team.

The Manchester Case Management Team’s models is a dedicated and co-located multi-disciplinary team established and led by general practice.

The service provides targeted support for those people in Manchester with the most complex needs and the highest risk of hospital admission; the 5% of the Manchester population with the highest risk score under the Combined Predictive Model.

The MCM team proactively support people identified, through a full review of a person’s clinical and non-clinical support needs, person centred and strengths-based care planning, assignment of dedicated key workers, regular multidisciplinary care reviews and implementation of support plans, Communities of Practice. Local delivery of clinical, mental, and social care services, focused on what matters to the person and their carer, are supported by rapid access to specialist advice, diagnostics, and clinical/ professional opinion.

The MCM team is a planned care / same day primary care service and will not provide other urgent care services or out of hours services.

The social care role within MCM is based on a strengths-based approach, which recognises what is important to the person. Employees will work as a part of the multidisciplinary team to support people to identify their goals, and then work alongside them as they make steps towards achieving positive outcomes in each of these areas.

This will include a wide range of activities, e.g., relationship building, Care Act Assessment, support planning, Carers Assessment, reviewing of support, safeguarding enquiry, provision of a key worker function, liaison with partner agencies, service development activities and community engagement.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration.
  • Post qualified experience.
  • Enhanced DBS/Update service.
  • References required for the last 5 years of employment.
