Job Description
RIACT is currently in the midst of a winter initiative, promoting a home first approach, utilising our current resources and amending our current processes to improve service user journey. The RIACT teams main roles are to facilitate safe and timely discharges, through use of intermediate care services or long term services. Following discharge from hospital it is vital we continue to provide ongoing reviews to support an individual through their recovery process and determining if any longer term services are required. As a Social Worker within the RIACT team, it is imperative that you would be able to work alone, whilst also alongside a very hardworking and dedicated team. You will have sound ability to assess and individuals needs, using a person centered, strength based approach. You will have sound literacy skills and ability to use the recording system to a high standard. You will have a good understanding of the Care Act 2014 and hospital discharge processes.
