– Right to work within the UK

This role involves assessment in a hospital setting or discharge to assess settings. We cover any hospital where there is a Cheshire West resident needing assistance. We have primary bases at Ellesmere Port and Winsford together with hot desking space available at Countess of Chester and Leighton Hospital as these are the 2 main hospitals we support. The role is hybrid in nature so workers can also work from home as part of their working week. This vacancy has been approved by Panel under Ref 541

The Hospital Team supports an MDT approach to discharge. The role of the social worker is to support discharge planning, risk assessment, safeguarding, mental capacity assessments, best interest decisions and Care Act Assessments for people who need support on leaving hospital. There is a Home First approach with discharge to assess in place.

You must have:

• A professional qualification in social work

• Social Work England registration.

• Post qualified experience.

• Enhanced DBS/Update service.

• References required for the last 5 years of employment.
