Job Description

3 days in office, 2 days working from home per week. Three positions available, 1 within the Winsford team, 1 within Ellesmere Port team and 1 within the Chester Team

Involves visiting families in their home

Hold a portfolio of cases

1 Co-ordinate a multi-agency Team Around the Family assessment , lead the development of a robust tailored multi-agency plan of intervention to address identified need, monitor progress against that plan and present a formally review progress as part of the TAF process

2 Co-work on cases alongside practitioners to provide the drive, challenge and support needed to effect the cultural change required within existing family support services to ensure effective full family assessment, reduction of duplication and holistic intervention planning.

3 Offer expertise related to own professional discipline or lead areas of work to highlight / reduce barriers and blockages with a view to achieving sustainable family outcomes to inform future service design

4 To be an advocate for families, who have been identified as being particularly challenging to existing service providers. Assertively working in partnership with the family and other agencies to identify and meet their assessed needs, developing pro-active solutions to prevent the need to implement foreseeable enforceable sanctions such as loss of benefits, eviction, loss of access to children, serving of anti-social behaviour orders

5 To undertake effective family engagement that will support families with personal / life issues, employability and skill development to enable them to resolve multiple and complex problems thus assisting in reducing workless households, bring about change in learned behaviours within families and ensure children are school ready

6 Support the development and maintenance of effective recording systems to enable the preparation of reports to measure effectiveness of individual interventions and contribute to the further development of the intervention planning and reviewing process.

7 To establish and enable effective working relationships with statutory and voluntary agencies to ensure a high standard of service to families, guaranteeing there are clear referral pathways into Integrated Early Support and appropriate services for those stepping down from the service as part of a TAF plan.

8 Ensure good communication and liaison is sustained throughout the Multi Agency Team Around the Family assessment and with relevant external partners, so that appropriate resources/finances may be identified and deployed for the benefit of the individuals and their families

9 To prepare internal and external reports, maintain accurate case records, evaluate the services and contribute to the policy / planning processes within the team, involving service users and potential users to ensure their views are captured and heard

10 Current awareness of and compliance with the statutory requirements, departmental policies, inter-agency protocols, equality and diversity and other regulations and procedures in order to ensure statutory and Departmental requirements are met

11 Be responsible for the health and safety of, self, colleagues and service users and the health and safety of equipment and operations within the team in accordance with the Health and Safety at Work Act. You will need to operate within the Council’s lone working policy

To prepare for and participate in supervisory, appraisal and training processes to ensure that you are a reflective practitioner whose practice is relevant, outcome focused, inquisitive and of a high quality

12 To work flexible hours, including evening and weekend work as required according to service needs

You must have:

  • Enhanced DBS/Update service.
  • References required for the last 2 years of employment.
