An exciting opportunity has arisen for a Social Work role that will focus on NHS Continuing Healthcare. The successful candidate would have recent experience and a thorough working knowledge of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care July 2022 (Revised) as well as The Care Act (2014). Good assessment skills and an ability to identify what care needs fall within the responsibility of the Local Authority will be required for this role. You will work within an MDT approach, undertaking Social Care Assessments and undertaking Decision Support Tools for individuals the NHS and Local Authority have identified as requiring consideration of CHC Eligibility. Social Workers will be working from home for the majority of the time, however visits to individuals will be throughout Lancashire. It is essential that you have good working experience working with Liquid Logic as this project does not afford time for training beforehand. This post is initially for 8 weeks, however there may be a possibility for further work.

You must have:

  • A professional qualification in social work
  • Social Work England registration.
  • Post qualified experience.
  • Enhanced DBS/Update service.
  • References required for the last 5 years of employment.
