Job Description
SPA- triage for hospital – discharge. Ability to apply the eligibility criteria under the Care Act. Ability to complete Continuing Health Care Assessments / Reviews. Ability to complete Mental Capacity assessments in line with the MCA 2005. Previous experience or knowledge of hospital discharge pathways. See attached JD for full details.
