Job description
ain responsibilities expected of the social worker is to practice the discharge to practice model, arrange services for the recommended discharge pathway to meet the assessed needs for patients prior to discharge from hospital. and undertake reviews. The worker is expected to have knowledge of legislations required to implement the Local authorities responsibilities
Responsible to:
Hospital Social Work Manager
Key relationships/Functional links with:
Responsible for social workers and to deputise at time for the manager

You must have:
- A professional qualification in social work
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
