Job Description

Experienced social worker required to support busy adults learning disability & autism service.

> Care Act assessments & Reviews 
> supporting safeguarding investigations, very complex cases, Mental Capacity Assessments (MCA) and Best Interests Decisions as part of the MDT
> management of high complexity and high- risk cases in conjunction with the MDT and social work colleagues
> Complex and high cost Transitions cases- supporting planning, review and post 18 support
> Supporting with annual review of support packages with a real focus on whole house/ supporting living settings
> supporting the progress relating to access to employment opportunities/ supported employment schemes
> The role is vital in ensuring that timely assessments and annual reviews are maintained whilst also supporting in the development of new working practices within the service.

> Must have experience of working with adults with learning disabilities & autism, and have a good understanding of the need for relevant reasonable adjustments, especially regarding communication.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration.
  • Post qualified experience.
  • References required for the last 5 years of employment.
