Job Description

We are looking to recruit 3 talented and effective officers in these roles in order to bring about the necessary change across the system to secure sustainable and effective person-centered services. Ideally you will have good knowledge and understanding of Adult Social Care and the key national strategies driving change in our sector. You will report directly to the Programme Lead and have regular interaction with senior executives representing the Local Authorities, Clinical Commissioning Groups and Health Trusts and emerging Integrated Care System across the Cheshire & Merseyside. You will be part of the sub-regional Programme Team and will ensure that there is good communication of the change programme within the team and across all partners. You will provide project management support for a range of transformation projects, in particular you will be required to support service planning and transformation for Age Well, Live Well and Independent & Fulfilling Lives. • JD and PS state LCR (Liverpool City Region), however sub region now spans Cheshire & Merseyside

PRIMARY PURPOSE OF THE JOB:

The postholder will be responsible for championing Change and Transformation of Adult Social Care and Health services across the Liverpool City Region. Innovation, efficiencies and continuous improvement will be the driving themes. Service excellence and efficiency savings will be the measure of success.

MAIN AREAS OF RESPONSIBILITY:

§ The management and implementation of projects and initiatives to support the delivery of the Change and Transformation Programme.

§ To prepare and maintain simple project initiation documentation and project plans, to include:

o the approach to the project and its segmentation into work packages

o the project scope, goals and deliverables

o project timescales

o project resources

o project organisation structure and resource needs

o data management approach and locations

o Communications approach and reporting.

§ To proactively manage changes in project scope, identifying potential risks and contingency plans and escalate as / when appropriate.

§ To build, develop and grow any business relationships vital to the project.

§ To work within the defined governance structure and framework, including the production of progress reports / presentations as required.

§ To manage project team and stakeholder involvement, communication, coordination and issues.

§ To contribute lessons learned and feedback throughout the project on any improvement opportunities or experience relating to the project.

§ To develop best practices and tools for project execution and management.

§ To assist in the development of needs analysis, service and specification design (which secure best value and deliver quality outcomes) and preparation of tender documentation, in liaison with the Commissioning and Contracts Managers and the Procurement Services.

§ Manage the effective performance and use of resources of projects, including monitoring budgetary information in liaison with the Performance and Finance Manager

§ To work in partnership with Clinical Commissioning Groups, acute and community health providers and other statutory, independent and third sector partners to ensure effective commissioning of health and social care services.

§ To actively and positively promote Liverpool City Region with external organisations and agencies, as well as retaining a high profile for the Change and Transformation Team within the LCR.

§ Ensure that appropriate arrangements are in place to enable a positive work/life balance in terms of personal workload.

§ Be responsible for ensuring that all duties and responsibilities comply with all statutory requirements and with policy and procedure.

§ Undertake, where required, other responsibilities and duties including work related to all areas of business, on behalf of the LCR, where this is commensurate with the grade of the post.
