Job Description

Herefordshire Council’s Adults & Communities Care Act Assessment Team (CAAsT) is seeking an experienced Locum Senior Practitioner to join their team based at Plough Lane, Hereford, covering the wider Herefordshire area.

This full-time role (37 hours per week) focuses on short-term assessment and support planning with service users who have recently left hospital via Discharge to Assess pathways and may require ongoing services, information, advice, and signposting. The work is fast-paced and deadline-driven, making it ideal for a practitioner who thrives in a dynamic environment.

Key responsibilities include carrying out Care Act assessments, overseeing duty for the team, managing allocations, triaging referrals, and signing off assessments and workflow. You will provide formal and informal supervision to colleagues and may hold a small caseload. Experience across hospital discharge, Continuing Healthcare, Mental Capacity Act (2005) assessments, mental health, learning disability, physical disability, and older people is particularly sought.

Candidates must be qualified social workers registered with Social Work England, with a strong knowledge of adult social care practice and legislation, excellent organisational and decision-making skills, and a person-centred, strengths-based approach. As Herefordshire is a rural county, the ability to travel across the area is essential.
