Job Description
Somerset Council is seeking an experienced Adults Social Worker to join a dedicated Learning Disabilities Review Project based in Taunton (TA1 4DY), operating on a hybrid basis with a minimum of two days per week in the office. The role offers £27.78–£38.00 per hour, starting 10 August 2026.
This is a specialist post focused on undertaking Care Act reviews and reassessments for adults with learning disabilities. You will carry out face-to-face visits across Somerset — in people’s homes, supported living, residential settings and community environments — gathering and analysing evidence from multiple sources to produce audit-ready assessments, support plans and case recordings.
Key responsibilities include determining eligibility for statutory reviews or full reassessments, identifying Continuing Healthcare (CHC) considerations, participating in multidisciplinary funding discussions, and delivering high-quality, person-centred, strengths-based practice throughout.
Essential requirements: a recognised Social Work qualification (BA or MA in Social Work or equivalent), current registration with Social Work England, significant post-qualifying Adults Social Care experience, and strong Learning Disability practice experience. Experience of Care Act reviews, reassessments, CHC processes and defensible decision-making is essential. A valid driving licence and access to a vehicle are required, as travel across Somerset is necessary.
If you are an experienced Adults Social Worker with a strong LD background and the ability to work autonomously at pace, we would love to hear from you. Please get in touch today.
