Job Description

Lincolnshire County Council is recruiting a Personal Adviser (Leaving Care Worker) based in Sleaford, Lincolnshire (NG34 7EB), on a temporary contract paying £27.33–£35.77 per hour via Prism, starting 20 July 2026.

Leaving Care is a statutory service supporting Care Experienced Young People aged 16–25 to live fulfilling, independent lives. Working within the Lincoln area, you will hold a caseload of young people still in care and those who have recently transitioned to adulthood (16–21 focus), developing high-quality, aspirational Pathway Plans using the Signs of Safety model and the Valuing Care tool.

Key responsibilities include building trusted, relationship-based support with young people and their networks; completing statutory face-to-face visits locally and nationally; coordinating multi-agency planning and delivering targeted interventions across accommodation, health, education, employment, training, finances, and life skills; acting as Lead Professional where appropriate; and participating in a duty rota to support young people in crisis.

Candidates should have experience supporting young people aged 16–25, a sound understanding of safeguarding and risk management, and the ability to develop impactful Pathway Plans. An NVQ Level 3 or equivalent is preferable. A full driving licence and access to a vehicle are essential, as is availability for national travel.
