Job Description

Warrington Borough Council is seeking two experienced agency Social Workers to join the Transfer of Care Hub (ToCH) at Warrington Hospital. This is a fast-paced hybrid role focused on supporting safe and timely discharges from hospital, working collaboratively with health and multidisciplinary partners to assess needs and reduce delayed transfers of care.

You will carry out assessments and work alongside health colleagues to facilitate smooth, person-centred discharge planning. The role is full time, 37 hours per week, with an expectation to be office-based approximately three days per week.

Recent experience in a hospital or Transfer of Care setting is essential, along with strong assessment, communication, and decision-making skills. Knowledge of the Mosaic social care system is desirable. Screening requirements include three years of references and gaps in employment history checked.

Rate: £30 per hour. Start date: 27 July 2026. End date: 02 November 2026.
