Job Description
Herefordshire Council is seeking a qualified Social Worker to join the Young Adults (Transitions) Team in Hereford, supporting young people aged 18–25 as they move from children’s into adult social care.
In this role you will complete strengths-based assessments, identify eligible needs under the Care Act 2014, and develop person-centred care and support plans that promote wellbeing and independence. You will also undertake Section 42 safeguarding enquiries, Continuing Healthcare (CHC) assessments, Mental Capacity Act assessments and best-interest decision-making, and contribute to court work as required. The role involves close collaboration with young adults, their families, and multi-agency partners to ensure safe and positive transitions into adult services.
Candidates must hold a recognised social work qualification and be registered with Social Work England. Experience of transitions, adult safeguarding, or preparation-to-adulthood work is highly desirable.
This is a locum position based at Plough Lane, Hereford (HR4 0LE), paying £23.65–£31.97 per hour via the Prism portal. Available from 22 June 2026. Contact us today to find out more or to submit your details.
