Job Description
Candidates would be working with the Adult and Older People’s Mental Health team based at West Park Hospital, Darlington. The team is fast paced, working with individuals with varying levels of complexity, we work alongside Secondary Mental Health Services with an MDT approach to support for individuals, to improve their well-being and maintain their mental health. The role requires the candidate to complete assessments under the Care Act 2014 as well as other duties outlined within the job description and skills section of the advert. There is an expectation that the candidate will hold a case load, complete annual reviews and contribute to the office duty rota, at times supporting the team with crisis situations which require joint working.
