Job Description

The candidate will need to be an experienced social worker with a mental health background and hospital discharge.

Core tasks and duties as follows:

  • Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge

  • Some mental capacity assessments, BI decisions and safeguarding as required

  • Understanding of Section 117, Attending 117 meetings, ward rounds, professional meetings

  • Participation in duty rota

  • Car driver preferred 

  • LAS (Liquid Logic) experience preferred Please note: This is a long-term temporary position with an immediate start available — perfect for experienced professionals ready to make an impact without delay.
