Job Description

2 x social workers required for a hospital discharge team and post discharge assessment team. The hospital team covers Huddersfield Royal Infirmary and Calderdale Royal hospital, completing hospital discharge assessments. Cover may be needed in the post discharge assessment team if needed, who assess and review those people discharged from hospital  into the community, whether they went home or into a care home placement. both 37 hours a week. 

Please note: This is a long-term temporary position with an immediate start available — perfect for experienced professionals ready to make an impact without delay.
